What did you want to be, do or have when you grew up? What has gotten in the way? What can we help you with in order to re-IGNITE those dreams you have? Are you a child still, with big dreams, but struggle knowing how to get from here to there? For many of our clients, physical and/or mental health issues may have prevented them from accomplishing all that they are capable of, and many have given up on their dreams. We find that most people just need some help figuring out what they are passionate about, and need someone to come alongside of them to manage details and DISCOVER capabilities and PASSIONS in their recovery JOURNEY. Community-based services can help your dreams be re-FUELED, re-IGNITED, and re-INVENTED for YOUR UNIQUE NEEDS.

 
 

Case Management

Case Management

What is Case Management?

Case Management services assist adults, adolescents and children who need an advocate to coordinate their care and services, refer to resources, and who have limited ability to obtain or arrange services for themselves. The goal of the program is to assist people with mental health struggles in obtaining the basic services required to live as independently and passionately as possible in their communities. Our case managers can also link and coordinate care to traditional or holistic options for you or your child.

Case Managers

Case Managers coordinate services and advocate for the person served. The goal of Case Management is to ensure an individual is receiving the support they need while working to increase the individual’s ability for self-support and self-advocacy. Case Managers can assist with the following:

  • Maintaining eligibility of coverage

  • Filling out various forms and paperwork for services

  • Housing and financial resources

  • Vocational or educational resources

  • Social and interpersonal supports

  • Health and medical supports

  • Recovery resources


CBRS: Community Based Rehabilitative Services

CBRS can be for Children, Adolescents and Adults where medically necessary

CBSR, formally known as PSR, consists of a variety of mental health services that are focused on promoting recovery and improving quality of life for adults, adolescents and children who have been diagnosed with a mental health condition requiring substantial support and wrap-around care. CBRS can help develop trade and life skills.

Generally services are provided in the client’s home or community and may include:

  • Symptom management

  • Coping skills

  • Communication skills

  • Problem solving skills

  • Social skills

  • Community Living or Trade skills

  • Crisis support

Certified Peer Support Services

Peer Support

Peer Supports come alongside of you and use their lived experience to help act as a peer coach and mentor in your own personal recovery story. CPSS is a form of community support service in which a Certified Peer Specialist works with you or your 18+ loved one to assist in regaining balance and control of your life and to support recovery. Peer Support assistance can take a variety of forms such as providing information about services or self-care, coming alongside of you to get out into the community, supporting you in the development of coping or recovery skills, and facilitating access to services and resources.

Certified Peer Specialists are an integral part of your treatment team, yet they can also assist in a way that is distinct from existing traditional mental health services. In addition to the utilization of their specific training and backgrounds, traditional knowledge and competencies in providing support, the Certified Peer Specialist relates practices from her or his own lived experience toolbox and experiential knowledge. Your Peer Support is someone who may have gone through something similar to your own struggles, and wants to help you reach your recovery and life goals. This factor allows for the Certified Peer Specialist to work from the context of recovery, utilizing language based on experience and ‘street smarts’, rather than clinical terminology.
